A histerectomia total por videolaparoscopia sem o uso de manipulador uterino tem se consolidado como técnica factível e segura, tanto em doenças benignas quanto, especialmente, em contextos oncológicos selecionados. Estudos observacionais e comparativos demonstram taxas de complicações semelhantes às técnicas convencionais, com a vantagem de evitar trauma cervical e vaginal. Em oncologia ginecológica, evidências recentes — incluindo ensaios randomizados e meta-análises — sugerem que a manipulação intrauterina pode estar associada a maior risco de disseminação tumoral e fatores patológicos adversos, como invasão linfovascular. Dessa forma, a técnica sem manipulador uterino representa alternativa tecnicamente válida, com potencial benefício oncológico, devendo integrar o arsenal do cirurgião ginecológico minimamente invasivo, sobretudo em casos de endometriose profunda, câncer de endométrio e câncer cervical inicial.
